Support Coordinators assist people to build the skills they need to understand and maximise the use of their NDIS plan.
If your NDIS plan includes funding for support coordination, a Support Coordinator will work with you to ensure you access a mix of services and supports that help you maintain relationships, manage the services you receive, live more independently, and be included in your community.
Your Support Coordinator will help you get the most out of your NDIS plan, working with you in these key areas.
Connection – Assisting you to develop knowledge, experience, and connections with your community and with broader systems of support.
Support design – Working with you to understand NDIS plan funding and its purpose. By getting to know you and your skills, your Support Coordinator can help you identify what you want from services and design support solutions to help you reach your goals.
Establish supports – Assisting you to consider and identify your options for support, and then linking you to broader systems of supports. Your Support Coordinator may also create a support and action plan to help you implement your NDIS plan.
Crisis – Your Support Coordinator will help you resolve points of crisis and develop capacity and resilience in your network.
Coach, Review, Reflect – You’ll be coached through any challenges and helped to review how well your plan is working for you. By reviewing outcomes, your Support Coordinator can help you reflect on your needs and take action to continually improve your plan.
